New Patients
To better serve our new patients we have made forms available for you to print off and complete at home prior to your appointment. This will help expedite your check in process the day of the appointment. Along with the New Patient forms please be sure to have the following.
- An updated Insurance card and referral
- Driver’s license
- Social security number
It is especially helpful to our providers if you are able to bring your medical records, a list of your current medications, and any recent diagnostic test results.
Medical Release Requests
If you are needing your medical records released to another medical office please fill out the Medical Release form and either bring it into the office or fax it to us at 910.221.9220
